Physical Therapy Forms Information

Over the years, a patient will visit a doctor or any medical practitioner multiple times.  Most patients just stick to one general practitioner while others might need to consult with specialist for other diseases and illnesses.  This is where important forms like physical therapy forms come in.

Another of the physical therapy forms to be submitted is the health information form.  A physical therapist conducts an initial evaluation with the patient to get a clear picture of the patient’s health background. This form seeks to identify any underlying medical problems that might complicate any physical therapy treatment plan that the therapist might suggest.  This form will eliminate any treatment plan that might aggravate the patient’s pre-existing medical conditions.

For disclosure and privacy practices, there are physical therapy forms for that purpose.  This form seeks to outline the conditions on how a patient’s medical information might be used, disclosed and how one can get access to the medical records.  This form assures the patient that his or her information will always be confidential.

The Health Insurance Portability and Accountability Act or HIPAA protects the right to privacy of patients.  All information found in physical therapy forms and in any medical records is protected by Federal Law.  Requesting for the release of any records needs to be accompanied with an authorization form which has to be duly signed by the patient.
